1Referral RequestSchool staff (speech pathologist, admin, or school psychologist) completes the district referral request form to have a student added to Embrace.

2EmbraceStaff member (that made the request) gets an email notification from Special Ed team letting them know that the student has been added to Embrace.

3Meeting Set
Staff member listed above or current case manager contacts the family and team to schedule the meeting and puts this date on Google Calendar and invites all team members including the parent.
Admin designates the potential case manager to attend the meeting if the student is not yet identified as disabilied.

4Interpreter Request

5MeetingMeeting is held.

6Paperwork
- Applicable boxes are checked
- Applicable forms signed
- Signatures uploaded into Embrace
- All completed original paperwork is sent to the SPED department at Central Office with all necessary signatures and applicable boxes checked

7Meeting Follow Up
- Parent is given a copy of all completed paperwork
- If the student is designated as limited English proficient (regardless if they are getting services or not) parent must receive paperwork in their native language as well as in English
- All staff who service/work with the student need appropriate paperwork (to be able to provide the support needed)
- Transportation is contacted if needed (by the building)
