Rachel - Consent Form Check
Every day, you want to check to see if the clients who have a first session tomorrow have completed their consent form.
To do so, check each clinicians schedule and look for "First Visit" tags on the sessions. Note if it is a consultation, you can ignore it, we are just looking at sessions.
If you find any first visit tags on a session, click the session and click "view" to see the clients profile. You want to scroll down to the intake form section on their profile, and see if the intake form is signed.
If it is signed, it will have a date/time that it was signed under the "status". See an example below of what a signed consent form looks like:
If it has not been signed, the status will say "in progress"
See below for an example of what an incomplete consent form looks like:
If the form is complete, move on to the next first session.
If the form is incomplete, following the following 2 steps:
1) Resend the consent form to the client
To do so, under "online intake form" click "email" then be sure to select the consent form for the correct therapist and hit "send now"
2) Inform the therapist. Email the therapist something like the following email:
Subject Line: Incomplete Consent Form for Tomorrow's client
Email body: "Hey (Therapist) - I just wanted to let you know that it looks like client #### has not completed their consent form for tomorrows session. I just resent them a new consent form in hopes that would prompt them to sign. Just make sure that before your session tomorrow that the consent form gets signed, or that you get it signed with the client in session tomorrow! Thanks so much"
