    Professional Teaching Standards

    Setting up an evaluation survey/PDH form

    School District Code for Forms (activity summary and evidence of completion):  040041000 - 26

      PD Activity Summary

      Instructor identifies the details outlining the PD and completes the Professional Activity Summary

      Summary is sent to the designated administrator for review with accompanying documents (agenda, session presentations materials, etc).

      These materials must be sent at least 2 weeks prior to the scheduled date of the event.

      Admin Review (CO or Building Level)

      Designated administrator does the following:
      • Reviews the Professional Activity Summary and provides feedback as needed to finalize for approval
      • Sends approved PD Activity Summary to the HR Department - PD Coordinator

      HR Department Review

      Designated HR staff member - PD Coordinator does the following:

      State Reporting

      District Level OP completes the following form for state reporting purposes (outside of district providers only).

      This form is due by June 30 each year.

      HR PD Coordinator will submit forms yearly to the state.

      Evaluation

      The evaluation link is sent to PD participants from the trainer (that the desginated CO admin/department generated).

      Participants have 5 days to complete the evaluation to gain credits for PDHs.

      District Level OP completes the following matrix once evaluation results have been gained and the form has been completed.  This is for internal and external providers.

      PDHs

      PDHs will be auto generated and sent to participants after they have completed the evaluation survey.

      Participants should print the form for their record and input the information into ISBE's website for their PD hours gained. 

      Professional development credit is awarded on a 1:1 basis – 1 clock hour of attendance = 1 professional development hours.

      Closing out the PD Experience

      Designated Admin/Presenter:​​​​
      • Completes the PD Ending Summary
      • Ensures that all documents related to the training are housed in the digital folder for review by the District Level OP
      • Emails District Level OP to close out the training session
      District Level OP:
      • Ensures that applicable matrices listed above (state and local) are completed.
      • Makes sure that all items are in the digital folder (see Audit form)
      • Prints a hard copy of all information discussed in the audit form and places in the orange folder for auditing purposes (one folder per event)
      • Completed orange folders are filed for auditing purposes
