1NotificationParent communicates a need to the specific building school personnel for the potential need for homebound services.
Or the family provides a doctor's order/note to the school nurse stating the student requires homebound instruction (ex: recent surgery, medical diagnosis,etc.). -
2Medical Review ProcessNurse/Health Coordinator reviews any doctor’s notes/orders (if applicable) provided to the school. If accepted, the family/provider does not need to fill out the medical certification form.

3State FormIf a family does not have a doctor’s order/note, then follow these appropriate contacts:
Elementary: Admin and/or School Nurse
Secondary: Admin, Counselor, and/or School Nurse would provide this form to the parent/guardian to have a provider complete and return to the nurse/health coordinator.

4Records Release
Records request form needs to be completed regardless by the parent/guardian to release/review applicable health records and in order to have communication with the doctor (ex: school nurse and provider).

5Applicable FormsBoth forms (step 3 and 4) once completed are shared with the school nurse and the Special Education Office Professionals at Central Office.

6Next StepsFurther direction will be provided from the nurse or SPED office professionals at CO to determine next steps (IEP meeting, 504 meeting, discussion of supports). Team then determines a plan for student’s education (remain in the current environment/homebound).

7Tutor Assignment
SPED office professional will assign a homebound tutor if one is warranted.
Under development: Elementary/Secondary Roles and Responsibilities
Homebound tutor will be provided with contact information for the student’s instructors/family. -
8ServicesA mutually agreed upon time/place is set between the tutor and the family. Students typically have 5 hours aweek of instruction from the homebound tutor.
