Student Transfer SOP

1NotificationSchool administration/clerical staff is notified that a student will be transferring schools (outside of the school district/D100).
- This could be during the current school year or at the end of the school year
- Additionally, if a student moves during the school year and the parent requests to keep their child in the district for the remainder of the year that is allowable (for that year only)
- However, as a district we will no longer provide transportation and the family would have to provide their own, unless it is a Homeless situation
- If the family intends to home school their student, encourage them to register with the ROE. Share the following resources:
- ISBE form isbe.net/Documents/87-02_hs_reg.pdf it can be completed digitally and emailed to ISBE or printed and brought to the ROE.

- If the family is transferring to a school out of the country, provide a copy of:
- Withdrawal form
- Transcript or report card, if available
- Last available grade report
- Medical records

2Move During School Year/Remain in D 100
- In the event a family selects to remain in District 100 for the remainder of the school year after they have moved out of district the following needs to be updated in eSchool to ensure accurate address information and notification that the child will not be attending the following year.

3District Google Withdrawal FormPlease complete this form for any student that will not be returning to the district (this applies to Pre-K to 8).
- This should be done for any moves during the school year or for students that will be leaving the district the following school year but as asking to remain within District 100 for the completion of the given school year.

4State Form (IL)The following state form should be completed (by clerical and administration) and sent to the school the student will now be attending to denote "Good Standing".
- This should be done at the time of transfer, not in advance to reflect current standing

5Drop FormA drop form should be completed in the office and communicated with appropriate parties. (see sample form)

6Records Released
- All cumulative records are released including health and immunization records, report cards, attendance records, standardized test scores (including EL information) / results, transcripts, birth certificate, and 504s (if applicable)
- Special education records and psychological tests will be released separately from the SPED department
- All records are sent directly by email or fax to the receiving school

7Incoming Student (requesting records)The following forms are sent by the district registrar to the student's prior school district when a new student registers. If a building does not have the information needed for a given student they can use these forms (without a parent signature) and request records again if needed from the child's prior school district.

8Illinois Transfer Form (Good Standing Form)When a student registers to attend a D100 school, they are required to complete the Good Standing Form Waiver.
- Schools do not need to wait to enroll a student until they receive the Illinois Transfer form.
If a student begins classes and then we receive a transfer form that they are not in good standing, contact the SIS Coordinator to have them unenrolled and notify the family that because they are not in good standing, they are unenrolled until their status changes. -
