1Student is unable to wear a mask
Staff explains to parent/guardian that if their student is unable to wear a mask, the mask exemption form must be completed by a physician (for the child to be present at school).
Staff will send the mask form to parent via email, mail, or pick up at school (parent preferred method). -
2Form Completion
Parent gives form to physician.
Physician fully completes form and returns to parent or faxes to school (school nurse). -
3Returned Form
Notification of completed form is shared with:
Building Administration
Nurse
Case Manager

4Staff Notification
Administration approves form in collaboration with the nurse (if needed).
Notifies case manager of approval.
Notifies transportation 815-544-1571.

5Form DocumentationNurse makes copy to add to student file (ability to access for those without Embrace).
Case manager uploads form in Embrace and notifies staff who work with the student that the form has been completed and approved.
