1Building Level MatrixBuilding level matrix is updated from the HR department/Quality Assurance Director (QAR) and is sent out to schools/departments before the school year begins (summer).
Administrative teams review the staff list/matrix and update where appropriate. If changes need to be made the HR department needs to be notified.
Administrative team designates on the matrix who will complete each certified staff member's evaluation. -
2NotificationCertified Staff Member is notified by their designated primary evaluator (Frontline Notification of Evaluation Form) by the first teacher work day of their evaluation cycle information and corresponding dates.

3Evaluation ProcessEvaluation Process is completed by the designated/primary evaluator and evaluation plan timelines/requirements are followed.
Timeline -
4Paperwork (Building Level)All paperwork is completed and provided to the certfied staff member within 5 days (after the post conference).
Completed paperwork includes:- Admin Signatures/Acknowledgment
- Teacher Signatures/Acknowledgment
- Ratings designated per Domain, for the Performance Review, for the Student Growth Component, and for the Summative Rating
Summative Evals- All summative forms are stored electronically. Annually, in April/May all administrators need to supply the HR Department (Attn: Director or Quality Assurance or Licensure Coordinator) a hard copy of the final summative form.
Student Growth Year/Form- These are maintained electronically via Frontline Professional Growth application.
Informal Observations are not sent to Central Office but rather are maintained electronically within the Frontline Professional Growth Application. -
5Central Office Review Process/RecordsSummative Evaluations are reviewed by the Quality Assurance Director.
Evaluations with areas of concern (basic or below in any domain) are given to the Assistant Superintendent of HR (Dr. Ady) for review.
HR clerical staff inputs evaluation data into school based matrices, Senate Bill 7 documentation, Business Plus, and Tenure Portability.
Once data has been entered in all of the areas above the evaluation is filed in the staff member's personnel file. -
