Student Safety Assessment (Level 1) SOP
Board Policy Reference: 4:170 and 4:190
SOP to facilitate the process to determine what a student says, writes, posts, or communicates is a legitimate threat to safety and security of themselves or others.
Level 1 Team (important responsibility):Identification of a potential student of concern- direct hear threat, social media, bullying
Assessment the risk of the student of concern,
Management of the situation- both student and given target
Providing appropriate support and interventions

1Threat DeterminationDetermine if what a student does is a legitimate threat to the safety and security of themselves or others.
First, if there is an imminent danger (weapon) address that safety concern before initiating this process. (Call law enforcement and follow district safety guidelines.)
Then, following the Reporting of Threat SOP, notify CO administration starting with Director of Student Services and Career Readiness or designee of the process of determining the threat to safety and security of themselves or others.
If it is an imminent threat continue student safety assessment if possible and/or in collaboration with the police department.
Collaborate messaging to the broader audience with the police department and CO administration (united messaging from school/police). -
2Threat to SelfIf the threat is determined to include a threat to self follow the following process and complete the following forms/documentation. (Spanish Version)
If that does not apply follow the outlined steps below. -
3ConsiderationsSchool team reviews the Level 1 Assessment Considerations to determine if they need to conduct a Student Safety Assessment.
If the team is unable to determine if a Level 1 Assessment should be completed, they should seek consult from another safety team member, Building Level Administrator, or Central Office Administrator. -
4Threat Determination
If yes, complete the Level 1 Student Witness Interview (Spanish Version)
Interview should be completed by a mental health professional and a school administrator.
Two members present during the interview.
Do not allow students to read the questions.
Do not delay the Level 1 Assessment
Ask questions through conversation or direct inquiry.
Explain confidentiality and its importance to the student-witness.

5Student Interview
Complete the Level 1 Student Interview (if possible at the same time) (Spanish Version)
Conducted by an administrator and a mental health professional.
Do not allow the student to read the questions.
Do not delay the Level 1 Assessment.
Ask the questions through conversation or direct inquiry.

6Staff Interview
Complete the Level 1 Teacher/Staff Interview.
Complete in person or over the phone
Can be completed without anyone present
Important to complete the interview promptly.

7Parent/Guardian Interview
Complete the Level 1 Parent/Guardian Interview. (Spanish Version)
Can only be conducted by a member of the Level 1 Student Safety Assessment Team.
This interview can be completed over the phone or in person.
Do not allow parents/guardians to read and answer questions by themselves.
Do not delay the Level 1 Assessment.
If the parent/guardian is not available or unwilling to participate, their information should not be included.
Ask questions through conversation or direct inquiry.
Document parent contact, attempt, and reason of omission.

8Assessment
Completed Level 1 Student Assessment Protocol
Team meeting to make this decision (multi-disciplinary team)
Admin reviews/facilitates the protocol with the team
Notetaker (other than meeting facilitator)
Compile all of the interview questions together
Team will determine appropriate actions, safety plan, and interventions
Administrator will maintain documentation (temporary folder/paper file) for any documentation.
If the team deems necessary, complete the Student Intervention and Supervision Form.
Intervention Plans should be shared with Case Managers- if this applies and reviewed on a monthly basis
