    360 Descriptors (Strengths/Attribute List/References)

    District 100 Perception Surveys

      Subject Invite

      Subjects will receive an automated email sent from Qualtrics to participate in an assessment.  

      Log in information:  district email address (ex: sbrenner@district100.com)
      Password:  A temporary password should be provided in the autogenerated email or you can select forgot your password.

      Evaluate Yourself

      Complete the 360 evaluation on yourself.

      Identify the 3 top attributes/strengths and provide a narrative explanation as to why.

      Nominate 3 Additional Evaluators

      These can be anyone from the following  list. 

      They can be a mix of building level and Central Office staff/peers.  

      The Superintendent will evaluate all building principals, assistant superintendents, and the communications department.  

      Assistant Superintendent of HR will evaluate all directors.  

      Building principals will evaluate APs, ADs, and Deans.

      Do not select/nominate the Superintendent, Assistant Superintendent of HR, or Principal if they will already provide you with feedback (based upon your role outlined above).

      Nominations

      Once nominations have been submitted the Superintendent/QAD will approve those nominations.  An email will be sent automatically to the designated evaluators to complete a 360 evaluation on you (that you nominated).

      Feedback

      You may provide the Superintendent or your Principal with feedback as well (each account is set up to do so) as an option.

      Peer 360

      Emails will be automatically sent to you, to provide feedback to the people that nominated you. Take time to fill out each one and to identify the 3 strengths and a narrative rationale as to why you selected that given strength.  Each person in the organization may have a different number of surveys to fill out based upon how many nominations they received.  The survey window is open between 1/28 and 2/11.  Reports will be run when the survey window closes.

      Reports

      Reports will be generated, provided to each subject, and at the upcoming leadership (or AP) meeting will be reviewed.  Individuals will be able to view their own specific feedback report.
