      Teacher Completes Field Trip Request Form

      Teacher submits Field Trip Request Form (at least 30 days prior to the trip).

      Form includes consultation with the school nurse.

      Principal Reviews Request

      • Principal reviews the request.
      • Building admin or designee enters in the field trip in the D100 Field Trip Planning Google Calendar
      • Principal looks into nursing needs/services/availability.
      • Principal either grants or denies the initial request.


      Assuming everything listed above is okay, principal signs off on paperwork and turns paperwork over to the OP.

      Teacher- Next Steps

      • Creates a field trip permission form and sends it electronically (Parent Square) to parents. Teacher sends print copies as needed. Copy of blank field trip permission form
      • Teacher lines up chaperones and ensures that they have filled out the volunteer form.  Ratio 1:10 
      • Notifies food service of the exact # of students needing a lunch (approx. 1-2 days prior).
