14.02 Group Health, Dental, and Vision Plans

Regular Full-time employees will become eligible for enrollment in CHPC’s group health, dental, and vision insurance plans following the completion of 30 days of continuous employment.

CHPC contributes to premiums for employee and dependent coverage for the health, dental, and vision plans offered to employees in an amount to be determined annually by the Staff Support Team, effective January 1 of each calendar year.

Employee contributions to the plans are paid by payroll deduction, which must be authorized in writing. Half of the monthly premium will be deducted each pay.
