20.03 Updates Required

After determination has been made regarding the employment status of the individual, the signed questionnaire must be filed with the Finance/Human Resource Office for record keeping purposes.
 
All continuing employees must complete an update to the screening questionnaire every three (3) years verifying the continuing validity of the information contained in the initial questionnaire, and updating the questionnaire with any new or previously undisclosed information relevant to any of the questions. The written and signed update must then be filed with the Finance/Human Resource Office.
 
If the employee is taking a position within a different CHPC Office or Ministry Area where contact with minors can now be expected, then an update to the screening questionnaire as described above must be completed at that time.
 
If there is a duration of 6 months or longer between ministry partner sessions, or if the ministry partner is taking a position within a different CHPC Office or Ministry Area where contact with minors can now be expected, then an update to the screening questionnaire as described above must be completed at that time.
