    Who Needs a Background Check?

    The following groups of individuals need background checks before they may begin ministry at CHPC.
    1. All employees
    2. Any volunteer with Children, Youth, Finance

    Authorization and Consent

    People apply for a paid position must complete an application form. The last page of the application form requests authorization to complete pre-employment background checks including criminal and drug tests (if warranted).

    If the signed application has all the information needed, the HR Department can request a background check without additional consent by the applicant.

    For volunteers, the background check will be sent to them for consent and to request needed information (such as social security number).

    Ordering the Background Check

    Background check is found in Simple Church on the Employee Profile.
    1. Select Order Background Check
    2. Select Package
    3. Ohio does not have a statewide option, so you need to select
    4. Select Plus (County) Package
    5. Depending on whether or not you have written consent from the candidate you will either select Consent already received or you will send to candidate for consent. For volunteer positions, you will send to candidate for consent.
    6. Step 2 of Background check - Insert Bill Reference Code and State. Billing Reference Codes are as follows:
      1. Children
      2. Youth
      3. Finance
      4. Employment
    7. Step 3 - Email to send to candidate - This email is sent by simplechurch, so it is good to send a separate email to the candidate or inform them to look for it and that it is a valid link.
    8. Step 4 - confirm your order and select Submit

    Update profile with background information

    1.Select Edit on employee profile2. Update Background Check Date with date you requested the background check.Save the update.

    Background Check Results

    The HR department will communicate the results of the background check to the employee's immediate supervisor.

    • The HR department will discuss any significant negative findings with the employee and provide an opportunity for clarification, if any.
    • Based on the outcome, HR department will give a written recommendation, indicating whether suitable employment is possible or not.

    Disposal of Sensitive Data

    The HR department will handle the background check reports and any other sensitive information obtained during the process in a confidential and secure manner.

    • Upon completion of the process, all documents will be securely shredded and disposed of properly.
    • Electronic data will be stored and disposed of in compliance with relevant laws and internal policies.
